CAMP - Comprehensive Asthma Management Program CAMP, Mobile C.A.R.E.'s core program, delivers high-quality asthma care, coupled with personalized education, to students at dozens of public and parochial schools and head start sites. By selecting schools as the primary delivery sites, Mobile C.A.R.E. is able to bring the doctor to the patient. Cost-effective and accessible, Mobile C.A.R.E. provides low-income families, with limited transportation and time, an opportunity to breathe easier. CAMP's comprehensive service model involves several steps, namely four core activities: - Identify and screen potential program participants;
- Acquaint prospective patients with the program and schedule an initial visit to one of the Asthma Vans;
- Initiate new patients into the program - conduct preliminary health screening, confirm asthma diagnosis, devise asthma management approach, provide asthma education and medication; and
- Provide follow-up care and education, while evaluating and adjusting treatment plan, as necessary.
